Instructions for filling CONSENT Form
To be filled by Parents/ Guardians
1. Click on the Link provided in our School Website or School App to open the form.
2. Download the form and take a print out.
3. Fill it completely (Incomplete form will not be accepted)
4. Sign it and scan it .
5. Send the scanned copy in the mail id: [email protected]
6. Student should carry the completely filled and signed form on the first day when he/ she is coming to School.
NOTE: Student will not be allowed to enter the School without the CONSENT form completely filled and duly signed by Parents / Guardians.
